Care3 - Aged Care Management

Care 3 is a private case management & placement service, focusing on services for older people residing in their own home or at a time of transition into residential care. We conduct a comprehensive assessment during the first visit to plan solutions.

A service package is recommended according to needs and clients can move between service packages as care needs change.

Care3 is able to step in to assess and make recommendations regarding what is required to support the family member, liaise with the family and implement agreed strategies.

Care3 can assist with the following:

• case management
• phone advice and support
• the most appropriate level of care and how to source that care
• availability of government funded services and private services and the best mix of these services
• information regarding respite care services
• residential aged care and placement services
• monitoring of wellbeing and general health.
